Central NorwichHome Visiting Service (St Stephens Gate Medical Centre, Castle Partnership,Lakenham Surgery, UEA Medical Centre and West Pottergate Medical Practice) hasbeen created to offer a service to their patients of Central Norwich. We arelooking for Clinical Practitioners to join our team providing high standards ofhealthcare to the patients of Norwich who are unable to be seen in ourPractices for a variety of reasons.
Main duties of the job
Working with the Practices of Central Norwich to carry out visits to our patients in their home, or a Care Home to assess, diagnose and/or treat for medical concerns; collaborating with our wider multi disciplinary teams of GPs, Nurses, Mental Health Support Workers, Physiotherapists, Dieticians, Administrative Support and Managerial Support.

Central NorwichHome Visiting Service (St Stephens Gate Medical Centre, Castle Partnership,Lakenham Surgery, UEA Medical Centre and West Pottergate Medical Practice) hasbeen created to offer a service to their patients of Central Norwich. We arelooking for Clinical Practitioners to join our team providing high standards ofhealthcare to the patients of Norwich who are unable to be seen in ourPractices for a variety of reasons.
Duties will include:
* Assessing patients in their home, or a care home with acute presentations of illness
* Diagnosing/treating patients in their own home
*Liaising with our wider practice team to ensure referrals and signposting to relevant agencies for the patients
* Attending Clinical Governance & MDT meetings within the Practices
* If of interest, supporting with Long Term Condition and Chronic Disease Management
* Identifying areas of Health Inequalities and communicating this to the Practices to enable service improvement
* Liaising with GPs with regards to patients who may need to be moved to a Palliative Pathway to enable care transfer
* Working as part of a Team, both the Practice Teams and the Central Norwich Home Visiting Team
* Supporting with seasonal/unprecedented needs such as vaccine delivery, if required - in capacity that is suitable to your personal skillset
