We are looking for an experienced Care Coordinator to join our team and support the development of a bespoke Community Care/Support Service across Buckinghamshire.
The role involves coordinating individualized care services for clients, enabling them to remain in their homes and communities.
Role: Care Coordinator
Location: Amersham, Buckinghamshire
Job Type: Full-time, Permanent
Salary: £24,000-£25,000 per year
What you will do:
* Ensure compliance with policies, procedures, and regulations.
* Conduct client assessments, reviews, and update care plans as needed.
* Liaise with families, medical professionals, and stakeholders for comprehensive support.
* Schedule carers, manage staff hours, and update recruitment files.
* Conduct staff supervisions, spot checks, and appraisals.
* Provide on-call cover and address punctuality or continuity issues.
* Promote equality, diversity, and inclusion.
* Deliver care in the community and transport staff if required.
* Identify and implement service improvements.
What you will have:
* At least 3 years of experience in providing care (required).
* NVQ Level 3 Health and Social Care
* A valid UK Driving Licence (required).
* Excellent knowledge of the Buckinghamshire area.
* Willingness to travel (up to 75%).
