Swift Primary Care Network are looking for an experienced administrator to join our PCN team. Working closely with the PCN Clinical Directors, PCN Finance Manager, PCN Network Manager and the PCN Practice Managers, you will play an essential role in ensuring effective and efficient delivery of healthcare services and support the Network Manager in the day-to-day running of the PCN. This is an exciting new opportunity for a motivated individual who can work independently and is flexible in responding to the developing nature of the role., Provide project support as and when required
Support Swift practices in delivery of the Covid-19 vaccination campaign
Support and engage with practices to deliver to deadlines where a PCN response is required
Co-ordinate PCN away days and liaise with partners to ensure smooth running on the day
Support the Network Manager and partners in the delivery of community and health outreach events
Opportunity to lead on project work where appropriate
Act as a point of contact for the PCN and cascade information as appropriate
Meet and liaise with Clinical Directors, PCN Network Manager and PCN Finance Manager regularly to prioritise tasks
Co-ordinate PCN meetings. Create and circulate agendas and produce information for specific areas of work where required
Support PCN Management team to ensure the plans are regularly reviewed and delivery against the plans is kept on track
Record minutes of all PCN meetings and keeping an up-to-date action log
Keep an up-to-date knowledge of the PCN DES and its requirements
Support the Finance Manager where necessary
PCNs are critical to the changing NHS and are the focal point of all locally developed planning and service delivery. Swift PCN was established in 2019 and covers 80,000 patients in South Bristol, across 7 GP practices: Armada Family Practice, Downton Road Surgery, Hartwood Healthcare, Hillview Family Practice, Grange Road Surgery, The Lennard Surgery and Bedminster Family Practice. Swift PCN are currently working on a range of projects including the PCN Paediatric Hub, Womens Health Hub, Childrens Asthma Clinic, Collaborative Lipid Fund project and various smaller projects, that require support., Swift Primary Care Network (PCN) is managed by a small team, made up of of three Clinical Directors, Network Manager and Finance Manager. We work closely with the 7 practices that make up the PCN. We also have a PCN Education Lead, PCN Pharmacist Leads, General Practice Nurse Lead and Community Innovation Lead that work to help build our relationships with external partners and support staff and project delivery within the PCN.
