1. Communication
· LiaisewiththeMDT(s)toensureaco-ordinatedserviceisprovidedtoinpatients.
· Participate in medical ward rounds as required, providing expert prescribing advice and contributingtotheworkofthe MDT.
· Counselandadvisepatients onuseofmedicines.
1. Analytical and Planning
· Undertake research and audit projects.
· Participate in CPD.
· Provide training for diploma students, trainee Pharmacists, nursing and medical staff.
· Take part in patient training sessions where available, providing information about drug usage and side effects, develop and deliver further training packages as services expand.

· Visit allocated wards and review inpatient prescription charts, checking for clinicalcorrectness according to current practice. Order non stock medicines for individual patients,ensuringcontinuityofsupply.
· Clinicallyscreenandfinalcheckalldispensaryprescriptionsaccordingtocurrent practice.
· Resolveanyprescriptionquerieswiththeprescriberandamendprescriptionaccordingly.
· Participate in dispensary duties e.g. prescription processing and dispensing, ensuring appropriaterecordse.g.controlled drugregisteraremaintained,whenrequired.
· Exercise professional judgment to resolve queries, referring to a more senior Pharmacist whennecessary.
· Monitorpatientmedication,ensuringsafeandeffectiveuseof drugs.
· Ensurecontinuityof supplyofmedicinestopatients.
· ParticipateinClinicaltrialsinvolvingmedicines.
· Always follow standard operating procedures and report to the line manager any occasions whereSOPscannotbefollowed.
· To carry out any other duties of a similar nature consistent with the responsibilities of this job description.
· Be proactive in risk management, ensuring that all clinical incidents are reported andappropriate action taken in conjunction with the Principal Pharmacist and/or the specialist riskmanagementpharmacist.
· Undertake any other duties that may be required from time to time as are consistent with the responsibilitiesofthegrade andthe needsoftheproject.

· Promotethesafe,appropriateandeconomic useofdrugs.
· Keep accurate patient medication and drug issue records using the Pharmacy JAC computer system.
· Generatereports/graphs/presentationsusingappropriatecomputersoftwarewhenrequired.
· Monitor drug expenditure within speciality, produce budget statements and drug usage reports,interpretandprovide analysisandrecommendations.
· Managethecosteffectiveuseofmedicinesincluding highcostdrugse.g.biologics.
· Take payment for prescription charges, issue receipts and refunds according to departmental procedures.
· Ensurethesafeandsecure storageofPharmacystockofdrugs andothermaterials.
· Monitor thesafeandsecure storageofdrugsonwards.
· Ensureallequipment has beenregularlymaintained.
· Ensureallequipmentisusedinaccordancewiththemanufacturersinstructions.
· Ensure any equipment found to be faulty/unsafe should be removed from use and reported tothe appropriateauthority.
