Job summary An opportunity has arisen for an enthusiastic and motivated individual to join our well established Heart Failure service Heart failure Clinical Nurse Specialist working with the context of a multi-disciplinary team supporting providing clinical assessment and management, health promotional support, teaching patient and staff around HF as well as be involved in research projects and support the development of the heart failure service. The post requires the successful candidate to provide clinical and health promotional management to a wide age range of patients who present with heart failure within the context of outpatient and in-reach clinical settings.
