Job summary Urgent Care Practitioner Yate and Frampton Primary Care Network are looking for an enthusiastic Urgent Care Team Member to join the friendly, well-organised, and supportive team at West Walk Surgery, for up to full-time hours. This post is available immediately, but we are happy to wait for the right candidate. This role would suit a Paramedic or a Nurse Practitioner who will work as part of our on the day team, assessing patients with acute undifferentiated problems. You will be supported by GPs and other urgent care practitioners in providing high-quality patient care. Hours of work - up to full time Pay depending on experience but likely to be in the region of 39-43K FTE Main duties of the job You will be joining a truly multidisciplinary team, consisting of 12 doctors, Paramedics, Clinical Pharmacists, a Mental Health Nurse, Physiotherapists, and a Social Prescriber. We hold daily meetings to catch up and discuss complex cases, weekly clinical team meetings for updates, and six-weekly clinical governance meetings. As a training practice for trainee GPs and a Core Teaching practice for undergraduates, we are committed to professional development and collaboration. The Urgent Care practitioner will be required to work autonomously in the practice and in the community (home visits), providing a range of services such as assessment, diagnosis, treatment, telephone triage and referring patients appropriately, as well as supporting the management team in the reviewing of clinical policy and procedure. Ensure that read codes are used effectively i. Ensure continuity of care, arranging follow-up consultations or reviews as necessary j. Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes k. Chaperone patients where necessary l.
