Job Summary
As an RN Nurse Assessor, you will play a pivotal role in our team and department, directly impacting the lives of patients by conducting face-to-face, needs-based eligibility assessments for individuals applying for Medicaid-funded personal care services. These services are provided in home settings or adult care/supervised living homes, offering essential support to those in need.
In addition, you’ll perform clinical evaluations for individuals with serious mental illness (SMI), intellectual disability (ID), or related conditions (RC) who are seeking admission to nursing homes.
