Please refer to the full Job Decription attached Attend endoscopy and appropriate surgical and oncological outpatient clinics to facilitate the identification of those patients with cancers of the HPB. Facilitate and act as resource/support at the patients point of diagnosis and may have to frequently contribute in discussing terminal illness/poor prognostic disease. Attend and contribute to multi-disciplinary meetings, both in the Royal Surrey County Hospital and any appropriate satellite sites to ensure optimal patient management. Ensure the provision of support both written and verbal for patients with cancers of the HPB enabling them to make informed choices and decisions at different stages of their cancer journey. Provide patients with cancers of the HPB, their carers and primary care givers with a contact within the Royal Surrey County Hospital to help coordinate their care appropriately and offer specialist advice regarding their treatment/condition. Act as a role model on the wards and within the clinic setting, facilitating the identification of the patients physical, social, psychological and spiritual needs and working with the multi-professional team to plan how these needs can be met. Use specialist knowledge to assess and interpret complex complications/conditions relating to HPB Cancers and instigate appropriate actions. To work in partnership with other members of the health care services both in primary and secondary care to enhance patient choice about their care and treatment. Able to work autonomously without supervision, managing caseload, providing patient focused, specialist care to complement the multi-disciplinary team. Whilst also working within team settings. Stay updated with trust procedures and policies to enable hands on participation in patient care. To assess, plan, implement and evaluate programmes of care for patients with HPB cancers. To ensure the use of evidenced based practice when caring for patients with cancer of the HPB. Identify areas where other nurse led initiatives could improve patient experience. To support the patient and their relatives through diagnosis, treatment and relapse. To work closely with the Palliative Care Team within the Cancer Unit and in the Community to provide expert care for patients with HPB cancers. To raise awareness throughout the Royal Surrey County Hospital NHS Trust of the role of the HPB cancer Clinical Nurse Specialist. To participate in the clinical governance process within the Trust and develop and manage a clinical risk management framework for HPB services. To network with other site specific/cancer care Clinical Nurse Specialists in the Trust. To link with HPB Clinical Nurse Specialists within the Cancer Network and act as tertiary link to ensure effective and timely management of patients treatment. Develop links with Clinical Nurse Specialists from all referring hospitals within the expanding catchment areas, to aid more effective communication. In conjunction with other members of the multi-disciplinary team develop Trust wide guidelines, protocols and identified pathways of care for the management of HPB cancers leading to the evaluation of multi-disciplinary care and the Clinical Nurse Specialist role. Participate in the Service Review process and help implement changes and recommendations with the Cancer Services, To participate in departmental audit projects, utilising results and outcomes to improve services including patient satisfaction studies. Maintain and develop the database of all patients with HPB cancer, working in conjunction with the audit collector. Participate in benchmarking both locally and nationally. To maintain and develop ones own professional knowledge and expertise to meet PREP and the jobs requirements. To actively share knowledge and expertise and contribute to the Trust as a whole To delegate appropriate responsibilities to enhance individuals level of practice and professional development To reflect as a practitioner, learning from experience and to promote reflective practices within the ward.
