You will workclosely with key stakeholders this role will be central to the delivery and successful implementation of the screening program and the optimal pathway. You will have clinical oversight of patients on the screening pathway and be responsible for facilitating onward referrals as recommended at the Screening Review Meeting (SRM); the role will be supported by a TLHC Navigator and the Responsible Assessor/Lead nurse for the programme.
