Job summary As part of East Cleveland Group Primary Care Network's multi-disciplinary team, the post-holder will work collaboratively to improve patient outcomes and support the PCN in achieving the ambitions of the NHS Long Term Plan. The post holder will work closely with a wider multi-disciplinary team (MDT), PCN member practices across Skelton, Guisborough, Brotton and Loftus to ensure patients with a cancer diagnosis receive the best possible care and service. The role will include communicating with patients to ensure they understand their condition, care pathway and treatment and to make sure that they are receiving the most appropriate support. As a newly established role, the post will be subject to review. Main duties of the job The Cancer Care Coordinator will work with the PCN central team supporting the delivery of NHS Englands Comprehensive model of Personalised Care and the Early Cancer Diagnosis Direct Enhanced Service. This pivotal role will support multidisciplinary teams and coordinate the pathway for patients with cancer, supporting public awareness of the earlier diagnosis agenda, as per the NHS Long Term Plan. The Cancer Care Coordinator will work with patients, ensuring they have the right support to understand their diagnosis and what the next steps are to help them during this distressing time, including personalised care interventions such as Personalised Care and Support Planning (PCSP) and Cancer Care Reviews. This is a patient-facing role, and the post holder will be responsible for providing support directly to patients and their carers. The post holder will act as a conduit for cancer patients within the PCN, liaising with secondary and tertiary care and the voluntary sector and, working closely with practice clinical leads and cancer champions where appropriate.
