The post holder will be responsible for facilitating multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures. The post holder will always adhere to the National Framework and guidance for Continuing Healthcare, NMC Code of Professional Conduct and work within established policies, procedures, protocols, and systems ensuring quality standards and performance as directed by the Complex Case Manager /Clinical Commissioning Manager.

Key Responsibilities:
1. Facilitate multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures, including face to face and virtual assessments/reviews.
2. Support the Complex Case Manager/Clinical Commissioning Manager and the wider team to ensure effective planning and management of the workstream.
3. Manage cases as directed by the Management Team, including clinical caseload management (end to end).
4. Ensure adherence to Continuing Healthcare guidance to maximize patient safety.
5. Support the pathway delivery for Personal Health Budgets within NHS Continuing Healthcare.
6. Identify and arrange services for individuals qualifying for NHS Continuing Healthcare.
7. Undertake investigations into eligibility for NHS Continuing Healthcare following challenges or disputes.
8. Act as Duty Clinician as required.
9. Maintain up-to-date data systems in line with standard operating policies and procedures.
10. Participate in ongoing professional development and clinical supervision in line with NMC guidelines.
11. Provide cover for other Nurse Practitioners as appropriate.
12. Maintain professional communication at all times.
13. Support the Clinical Assessment & Placement Team in achieving team goals and objectives.
14. Communicate relevant information, risks, and issues effectively.
15. Contribute to meetings, providing information and support as requested.
16. Deliver against objectives and performance targets, prioritizing workload and meeting deadlines.
17. Support training and induction of new staff.
18. Undertake research and development as directed.
19. Comply with all statutory and mandatory training requirements.
20. Carry out any other duties as reasonably required by the line manager.
