Daily Clinical Responsibilities: Patient Consultations: Conduct face-to-face, telephone, and video consultations, providing assessments, diagnoses, treatment plans, and referrals where appropriate. Manage a broad range of acute and chronic illnesses, ensuring that care is delivered in line with evidence-based guidelines and best practices. Prescribe medications safely, managing repeat prescriptions and working with the wider PCN team for prescription reviews. Chronic Disease Management: Participate in the management of chronic diseases, such as diabetes, hypertension, asthma, and COPD, ensuring that patients receive optimal long-term care. Collaborate with practice nurses and other healthcare professionals to deliver structured care plans for patients with long-term conditions. Preventative Care & Screening: Promote preventative healthcare measures, including immunizations, cancer screenings, and lifestyle interventions. Identify opportunities for early intervention and disease prevention through screening, lifestyle advice, and health education. Referral & Coordination of Care: Refer patients to specialist services when required, ensuring a seamless transition between primary, secondary, and tertiary care services. Work closely with multi-disciplinary teams within the PCN to provide coordinated care for patients, especially those with complex or multiple health conditions. Health Improvement Work: Health Promotion: Participate in health promotion initiatives to improve the health of the local population, including mental health awareness, smoking cessation programs, health screening and weight management support. Refer patients to statutory and voluntary agencies as appropriate. Contribute to PCN-led and practice-lead population health management strategies, helping to address health inequalities within the community. Public Health & Community Engagement: Engage in public health initiatives aimed at addressing the social determinants of health and reducing inequalities in access to care. Collaborate with local health and social care services, voluntary organizations, and community groups to enhance patient care and well-being. Quality Improvement: Actively participate in audit, research, and quality improvement activities, contributing to the continual enhancement of patient care standards. Implement evidence-based interventions to improve patient outcomes and reduce healthcare system inefficiencies.
