Job summary
We are seeking highly motivated and enthusiastic clinical pharmacists to join our clinical team. If you are an IP with experience working in General Practice (essential) we'd like to hear from you. The is a remote working role suitable for candidates looking for permanent work.
Main duties of the job
Using GP systems (emis and s1) you will support practice sites with medicines related tasks, this may include but is not limited to:
1. Discharge support (meds rec)
2. General med reviews
3. Supporting patient requests for acute and repeat meds
4. Structured Medication reviews
5. Safety audits

Job responsibilities
You will work as part of a multi-disciplinary team in a Onsite role, working with colleagues to help resolve medication-related issues, helping to utilise skill mix, improve patient outcomes and ensure better access to healthcare.
You will support practice staff with prescription and medication queries. Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.
You will support with prescribing practices across the PCN, providing clinical leadership on medicines optimization and quality improvement.
