Health and Wellbeing Practitioner Location: East Lothian, EH16 4EA Salary: £20,281 (pro rata of £25,352 per annum) Closing date: 5th February 2025 Interview date: 14th February2025 The Role Our client is looking for a part time Health and Wellbeing Outreach Worker to support the delivery of their Health and Wellbeing service in East Lothian. The outreach worker will work alongside two Health and Wellbeing Practitioners who will be predominantly based within GP practices in Musselburgh and Prestonpans to provide outreach support to people referred from GP practices across East Lothian. Are you a health and social care worker with experience of supporting people living with long term physical and mental health conditions to self-manage? Do you cope well supporting vulnerable people who experience significant psychosocial barriers and health inequalities? Do you believe people have strengths, are resilient and are experts in their own lives? Do you thrive working autonomously in the community setting? Do you have good solution focused coaching and group facilitation skills?
