Job Overview We are seeking an experienced Band 6 Occupational Therapist to join our community team in Marlow and Thame. This role focuses on elderly care, conducting community visits, and delivering person-centered therapy to enhance patients' quality of life and independence. Key Responsibilities Manage a community-based caseload, focusing on elderly care and rehabilitation. Conduct home assessments and implement interventions to improve safety and independence. Collaborate with multidisciplinary teams to coordinate comprehensive care plans. Provide advice and support to patients, families, and carers to achieve therapy goals. Document patient progress and maintain accurate records in line with trust policies. Key Requirements HCPC registration as an Occupational Therapist. Proven NHS experience in community and elderly care settings. Knowledge of current practices in community rehabilitation and home safety adaptations.
