This post holder will offer support to palliative patients with complex needs and will support the medical and nursing staff of the hospital and the community team.
The patient’s choice will be central to offering choices to patients about treatments ad paces of care. Creating positive and therapeutic relationships with patients and their carers is essential
This post is an integral part of the multi-disciplinary team within the locality. The post holder will attend the palliative care Gold standard meetings.
This post also provides education and training to the community and hospital nursing, medical, social work and AHP teams.
