Post Graduate qualification in Autonomous Practiceis essential and a non-medical prescribing qualification is also essential. Main duties of the job
To perform as an autonomous practitioner working independently acrossour PCN and with other health care professionals. Managing daily surgeries offace to face, online and telephone consultations to assess, diagnose and treatpatients with a range of acute, non-acute and chronic medical conditions,initiating direct referrals as appropriate.

Clinical responsibilities:
Work autonomously and be accountable for own professional actions.
1. Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.
2. Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans ensuring relevant follow up.
3. Discuss with registered GP and, where appropriate, make referrals to other health professionals in primary and secondary care.
4. Arrange hospital admission for appropriate cases.
5. Undertake telephone triage work, telephone advice calls and online consultations.
6. Managing and undertaking home visits including Care Home visits
7. Managing Palliative Care patients
8. Prescribe medication as necessary as a non-medical prescriber.
9. Maintain accurate record keeping in SystmOne/EMIS using templates and READ codes.
10. Undertake regular medication reviews and medicines management.
11. Request and interpret tests appropriate to patients needs.
12. Manage communications from hospitals and other providers Immediate Discharge Summaries acting on requests for further action, medication etc
