Pathways Coordinators are an integral part of the Outpatients team, looking after patients from referral to discharge, ensuring that the process is both efficient and effective. This is achieved by accurately processing and actively monitoring the flow of information throughout the patient pathway.
The role would suit someone who is good at multitasking, problem-solving with an empathetic nature, and with a passion for helping patients.
The Pathways team is currently made up of 6 staff members based in the Outpatients department at Dorking Community Hospital. As we are seeing the number of patients grow, we are looking to expand our team.
Main duties of the job
Pathways Coordinators are responsible for ensuring that the patient journey is both efficient and effective. This involves chasing reports, results, and other documentation with GPs, hospitals, and third-party providers to ensure these are in place prior to appointments.
When processing reports, the Pathways team will highlight any urgent findings to our clinicians to expedite patient care where required. The ideal candidate would therefore be someone who can remain calm and maintain a high level of attention to detail.
The successful applicant will be able to help manage the workload efficiently and effectively on a daily basis to facilitate the patients' care being as smooth and efficient as possible.
We will provide the training necessary to use our in-house IT systems; however, a good working knowledge of Microsoft Office would be beneficial. You will be required to maintain patient confidentiality at all times.

Job responsibilities
The role (and team) is an integral part of the patient pathway (referral to discharge) ensuring that the process is both efficient and effective. This is achieved by accurately processing and actively monitoring the flow of information throughout the patient pathway.
The Pathways team looks through each patient's referral carefully to make sure the clinician has all the relevant information about the patient's condition available to them on our electronic record system. This means the patients get the most out of their time with the consultant as all relevant test results and images are available for review.
We work with several NHS Trusts and independent hospitals in the local area and have our own in-house diagnostic services. A part of the Pathway role is to forward on results from tests to the patient's GP surgery as well as our consultants.
You will need to have excellent attention to detail and use initiative and good judgement to problem-solve and consider the best course of action to take for the patient.
