Job Title: Experienced Hospital Discharge Social Worker
Location: Hammersmith and Fulham
Rate: Up to £35 per hour
An exciting opportunity has recently become available for an Experienced Hospital Discharge Social Worker. They are looking for a new member to join the team in Hammersmith and Fulham.
What will your responsibilities be?
You will be enthusiastic to engage in adopting strengths-based, person-centered assessments. You will manage a varied caseload, which will include capacity assessments, best interest decision making, Care Act assessments, and being part of a duty rota. Additionally, you will deliver exceptional social care to the adults we are supporting and their carers, promoting choice and control as much as possible. You will also engage in outcomes-focused and preventative conversations which build on the strengths of people and their local networks.

Qualifications and Experiences:
To be successful in this role, you must have experience working as a Hospital Discharge Social Worker. You must also have Social Work England registration, be eligible to work in the UK, hold a full UK license, have post-qualified experience, and hold a degree level or equivalent qualification in Social Work.
