Deliver expert care by performing high-level assessments, agreeing on personalised treatment plans, and initiating injectable therapies. Empower patients to thrive, supporting them to achieve independence or reach their maximum potential through education, shared decision-making, and seamless collaboration across care teams. Work independently, with support from senior diabetes specialists, to achieve exceptional quality and performance standards. Collaborate with stakeholders across the system, maintaining high standards of care while delivering impactful education and training to both colleagues and service users.
