Advanced Clinical Practitioner for Frailty Services

We are looking for an Advanced Clinical Practitioner (ACP) to work within our community-based integrated service, with exciting rotational opportunities through:
1. Single Point of Access
2. Integrated Urgent Community Response (UCR)/Hospital at Home (H@H)
3. Acute Frailty Services, including:
1. Emergency Department (ED) in-reach
2. Frailty Same Day Emergency Care (FSDEC)
3. Inpatient Older People's Advice and Liaison (OPAL)
This role is open to both Allied Health Professionals and those with a nursing background who meet the essential criteria.

Job responsibilities
The Advanced Clinical Practitioner (ACP) will provide expert assessment, treatment and management of a case load, using complex communication skills, judgement, discretion and decision making. They will monitor and assist with improving standards of care, supervising, teaching and supporting colleagues providing skilled professional leadership. Where appropriate they will act as an expert in their field and lead and/or participate in initiatives to improve standards of care for the client group and assisting to advancing the Frailty ACP role.
The Advanced Clinical Practitioner is service driven, and the role is structured around the Department of Health Guidance across the four domains of Advanced Practice, those being:
1. Clinical Practice

3. Research

Person Specification
Qualifications
* NMC / HCPC registered professional
* Relevant MSc, supplemented by specialist training, experience and short courses.
* Independent non-medical prescriber (where applicable to role/professional group).
* IRMER (Imaging Referral by Non-Medical Healthcare Professionals) training complete.
* Non-medical authorisation of blood products training completed (as applicable to role/professional group).
* Completed advanced clinical practice MSc
* Advanced assessment skills qualification with evidence of clinical practice
* Teaching qualification
Knowledge and Experience
* Previous band 7 experience in setting where clinician will practice
* Previous leadership experience (band 7 minimum)
* Significant post registration experience in Frailty and Comprehensive Geriatric Assessment (Tier 3 competency training aligned to the core capabilities framework) - to the level of advanced practice
* Experience and evidence in developing practice
* Experience of supporting learning in the clinical environment
* Acknowledged expert in their field who is clinically credible
* Experience of conducting audit/research and implementing innovative practices
* Familiar with aspects of the NHS agenda and national directives and strategy relating to patient safety and quality improvement
* Understanding of the changing landscape of the NHS
* Experience of disseminating work through presentation/publication (Conference, presentations, publications)
* Previous experience in a practice development role or formal teaching experience
* Peer reviewed publication of research
* Evidence of reviewing articles for publication
