The Hospital Mental Health Liaison Team practitioners work alongside the multidisciplinary team within the acute hospital setting including medical and nursing teams, Community Mental Health Teams, Local Authority community services and Voluntary sector. The role will serve to identify and support patients who present within the acute hospital with a variety of mental health conditions alongside the potential of complex physical comorbidities, individuals can present in crisis and display challenging or vulnerable behaviours who require a comprehensive mental health assessment to support and manage mental health problems and to contribute to the discharge planning process. SW will have the opportunity to work across all ages from 16+ to older age and across both A+E and ward environments. We are seeking adaptable, flexible, innovative mental health practitioners who wish to develop their professional skills within a hospital setting. This will involve completing mental health assessments and the ability to effectively signpost while forging links with community organisations across health and social care. You will build and maintain effective relationships with health professionals in the general hospital settings to raise awareness with regard to individuals with mental health issues. You will be expected to work independently with confidence in order to make informed clinical decisions regarding appropriate signposting/discharge process. The role includes supportive observations, also support the 136 process in the A&E department.
