Job Details: Hep C Outreach Support - Primary Care

Hep C Outreach Support (Primary Care) - Fixed Term 12 Months
Working hours: 37 hours Monday-Friday
Location: Base location TBC
About the role...
An opportunity has arisen to support the Hepatitis C elimination agenda as part of the Waythrough Hep C team in a post hosted by the West Yorkshire Liver Care Network. This will initially be funded for 12 months with the opportunity to extend for another 12 months subject to funding becoming available.
Some key duties will include:
* As directed by Hep C Regional Co-ordinator and working within Primary care settings in Bradford, the role will ensure that Hepatitis C and other BBV interventions are being delivered to those who are marginalised from mainstream services and require support to access health and wellbeing interventions.
* You will raise awareness of Hepatitis C and other BBV with staff, mentors, volunteers and individuals accessing Primary care with a focus on those who are seeking refuge and asylum, sex working, complex, disadvantaged and struggling to access BBV testing and treatment.
* You will provide support to access help with substance misuse services, housing and wellbeing. Outreach will include working with ODN partners to deliver liver health checks on the accessible van and ODN nurse home visits.
* Working with ODN, Hep C Trust staff and peers you will ensure that those in need are engaged with other services and afforded equitable care with support from wellbeing mentors.
About you...
* You must have a valid driving licence and access to a vehicle or the ability to travel via public transport efficiently, to be considered for the role.
* Lived experience would be an advantage but not essential.
* You will need to have excellent communication skills, be able to raise awareness of BBV with those working in and accessing Primary care, experience of substance misuse and mental health issues, sound knowledge of Hep C and a passion for working as a team.

The salary advertised above demonstrates the full earning potential for this role, starting salaries will allow for pay progression within the pay band.
