Provide cross site leadership and clinical expertise in the delivery and development of pharmacy services to the Directorates of Nephrology and Renal Transplant. Developing anexemplary renal clinical pharmacy service. Lead across the ICS as a expert, upskilling Pharmacists' across primary and secondary care. Develop and manage the renal pharmacy team to ensure safe and effective specialist clinical pharmacy service for patients and staff. Work in conjunction with renal consultants, transplant surgeons and nurses in the nephrology clinics to identify and optimise patient co-morbidities and to refer to other specialties or services in a timely manner. Provide Consultant Pharmacist level input to patient care, promoting and providingmedicines optimisation for all renal in/outpatients and dialysis patients. Lead the strategic direction and implementation of a harmonised approach to pharmacy services to Directorates of Nephrology and Renal Transplant. Develop, manage, implement, and evaluate the renal pharmacy team services to theDirectorates of Nephrology and Renal Transplant Lead and/ or contribute to local, regional, and national nephrology workstreams which will focus on optimising the management of patients with kidney disease. Use innovation to support effective patient outcomes, safety and experience aligned with relevant national agenda. Work with external agencies to develop service delivery pathways to address unmet clinicalneeds in renal medicine Ensure the effective and efficient use of resources across the primary/secondary care interface and where necessary develop relevant protocols. Line manage, coach, and develop pharmacists working within renal medicine and transplant Develop and deliver education and training to pharmacy, medical, nursing and other staff related to renal medicines optimisation at both a local, regional and national level. To lead and maintain a portfolio of research, audit and service improvement within renal medicine across LUHFT.
