We have an exciting opportunity for a Case Manager to join our team. The Post holder will be largely based across Ayrshire within employability partner premises. Working within Salus Return to Work Services based in Beckford Street, Hamilton the Post holder will be required to work in a flexible nature across both health board areas.
The Role
As a Case Manager, your day to day tasks will include:
1. Working from multiple partner venues
2. Running drop in sessions for potential new clients
3. Engaging and liaising with existing / new referrers
4. Managing a case load of clients referred (approximately 80)
5. Supporting clients with presenting health conditions (predominantly mental health)
6. Undertaking structured assessments (face to face) to identify difficulties, deconstruct problems, collaborate on agreed actions and provide ongoing support with:Guided self-help techniquesSocial prescribing within the local communityReferring to therapeutic treatments where appropriate and managing the processMotivationEncouraging positive changeEnhancing engagement with work agencies / employers
7. Providing quality report output for clients and referrers in the form of Action Plans, Mid Reviews and Closure Reports
8. Measuring progress utilising an agreed suite of clinically validated tools
9. Maintaining accurate and detailed client records

Please note this is a fixed term post for 2 years. Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post.
Location and Working Pattern
This post is based in the community working with participants on a face to face basis. We are initially looking for this post to be based in our South Ayrshire venues however this could potentially change to alternative venues across Ayrshire dependant on contractual needs. The job holder will need to have a current valid UK driving licence with access to own vehicle.
The 37 hour working week will be Monday-Thursday 9-5 and Friday 9-4.30.
