Care Coordinator – Domiciliary Care
40 hours per week + on-call | £28,000 (rising post-probation)
Our client, a well-respected and growing provider of domiciliary care, is excited to be opening a new domiciliary care branch—and they're looking for an experienced and compassionate Care Coordinator to play a key role in its success.
This is an exciting opportunity for someone who thrives in a fast-paced, people-focused environment and wants to help shape a service from the ground up. The successful candidate will be central to delivering safe, high-quality care that enables people to live well in their own homes.
As a Care Coordinator you will work closely with the Registered Manager to ensure care packages are planned and delivered effectively, schedules are well-organised, and clients receive the consistent, person-centred support they deserve
.
Due to the nature of the role, candidates must hold a full driving licence, as regular travel within the community will be required.
Required Skills & Qualifications
* Level 3 (or above) in Health and Social Care
* Previous experience in the care sector – ideally in a Care Coordinator or team leader role
* A confident communicator with strong organisational skills
* A proactive problem-solver who can manage changing priorities
* Full UK driving licence and access to a vehicle

Key Responsibilities
* Meet regularly with service users to review and ensure care aligns with their individual care plans
* Train and assess care staff in the safe and effective administration and management of medication
* Coordinating care visits and rotas to meet service users’ needs
* Managing client referrals and care planning alongside the Registered Manager
* Supporting and supervising care staff, including conducting spot checks and field visits
* Handling on-call duties on a rota basis (shared within the team)
* Ensuring the quality and compliance of care provision in line with CQC requirements
* Building strong relationships with service users, families, and professionals
