Work as an autonomous practitioner with the integrated care team providing rapid and planned expert professional assessment and treatment interventions to patients in their own homes or other community settings to enable patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible. This will be achieved through a combination of hands on expert clinical skills and knowledge to treat, educate and support patients on a designated caseload and by working with patients and their carers to assess, plan and implement appropriate packages of care. Demonstrate expert clinical knowledge, health promotion and holistic assessment skills and will work with the patient and their carers to assess, plan, implement and evaluate their ongoing care and support needs in their own home or temporary/usual place of residence. Support the Operational and Clinical Team leaders in the delivery of best nursing practice, be involved in triage, allocation of work and leadership of other staff, manage team resources and deputise in their absence.
