Prepare reports and represent clients at Court hearings and Court of Protection applications. Make complex decisions and manage risk using professional judgment and evidence-based tools. Communicate effectively in high-risk situations and represent the department in multi-disciplinary forums. Conduct direct work with service users, carers, and families following safeguarding policies. Assess and navigate care for individuals with eligible social care needs, collaborating with health agencies. Plan and deliver services to meet assessed needs within financial constraints, monitoring support plans. Participate in safeguarding enquiries, capacity assessments, and best interests assessments, ensuring timely completion.
