Job Summary
We are seeking a dedicated Care Manager to oversee and coordinate care services for individuals in various care settings, including nursing homes, assisted living facilities, and home care environments.
Responsibilities
* Develop and implement individualised care plans based on residents' needs and preferences
* Supervise and train care staff to ensure high-quality service delivery
* Monitor and evaluate the effectiveness of care plans and make adjustments as necessary
* Coordinate with healthcare professionals, families, and external agencies to provide holistic care
* Ensure compliance with regulatory standards and guidelines in the provision of care services
Skills
* Previous experience in nursing or a related healthcare field
* Proficient in creating and implementing care plans
* Knowledge of senior care, dementia care, and medication administration
* Familiarity with various care settings such as nursing homes, assisted living facilities, and home care
* Strong communication and interpersonal skills to liaise with residents, families, and healthcare professionals
Job Types: Full-time, Permanent
Pay: From £36,000.00 per year
