Be person-centred and compassionate when caring for the person and those important to them. Ability to develop and evaluate care management plans alongside other healthcare professionals that meets the complex needs of the person, and those important to them. Proven understanding the barriers and enablers to good communication and how challenges can be overcome. Willing to line manage healthcare assistants. Ability to assess Healthcare Assistant clinical skills. Ability to undertake staff observational/support visits in the Community. The ability to co-ordinate care/prioritise patient needs.
