Are you a dedicated and experienced Qualified Social Worker seeking a fulfilling role in a supportive team environment? Look no further! We have an exciting opportunity for a Qualified Social Worker to join our Locality Team in Sutton.
As a Qualified Social Worker in our Locality Team, you will have the opportunity to work directly with individuals and families within the community, providing essential support and intervention. You will play a crucial role in promoting their well-being, independence, and ensuring their needs are met.
Responsibilities:
1. Undertaking assessments of individuals and families to identify their support needs and develop care plans.
2. Providing direct support, counseling, and advocacy to individuals, ensuring their voices are heard and their rights protected.
3. Collaborating with partner agencies, community resources, and healthcare professionals to coordinate and deliver holistic services.
4. Safeguarding vulnerable individuals, adhering to relevant legislation and local policies.
5. Regularly reviewing and monitoring support plans, adapting them to meet changing circumstances.
6. Maintaining accurate and up-to-date records, ensuring confidentiality and data protection.
7. Participating in team meetings, case conferences, and multidisciplinary forums to share information and contribute to collaborative decision-making.
8. Keeping abreast of relevant legislation, policies, and best practices in social work.
Requirements:
1. Qualified Social Worker with current registration with Social Work England.
2. Experience working in a locality or community-based social work setting, supporting individuals and families.
3. Sound knowledge of relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005.
