Children's Community Asthma and Wheeze Specialist Nurse
Our Children's Asthma and Wheeze service has a great opportunity for you to become part of our specialist children's community asthma and wheeze service. Previous experience and knowledge of managing asthma and wheeze is preferable; however, support and training will be provided on commencement of the role.
The children's community asthma and wheeze service provides specialist nursing provision for children and young people aged 2 to 16 years with asthma and wheeze in the community who have uncontrolled asthma.
The specialist asthma and wheeze nurse will undertake assessments and provide support to children, young people, and their families to safely and effectively manage their asthma to reduce ill health and hospital admissions. This is achieved by running nurse-led asthma clinics and telephone reviews as well as undertaking home and school visits.
The specialist asthma nurses work collaboratively with the specialist respiratory hospital team, GPs, practice nurses, and pharmacists to achieve a high level of support for children and young people with asthma and wheeze. They will also provide asthma training for primary care and education staff across West Hertfordshire to promote safe and effective management of asthma in the community.
You must hold a full valid driving licence and have access to a car for regular business purposes (unless you have a disability as defined by the Equality Act 2010).

Main Duties of the Job
To work autonomously to provide high standards of assessment and provision of care to children and young families referred with asthma and wheeze in line with the national guidance.
* To manage a caseload, prioritise and delegate.
* Demonstrate a professional approach to work, and act in accordance with the NMC Code of Professional Conduct at all times.
* To act on own initiative and in such a manner as to uphold and enhance the good standard and reputation of the Nursing, Midwifery Council.
* To undertake specific delegated responsibility for the teams in the absence of the Team Leader.
* To function as an expert clinical practitioner within the Children's Asthma and Wheeze Team, delivering direct patient care while managing, leading, coordinating, and overseeing nursing practice.
* To ensure efficient and effective utilization of all resources required to deliver optimum quality care to children, young people, and their families who access the Children's Asthma and Wheeze Nursing Service.
Job Responsibilities
* To work effectively with sound judgement when dealing with complex emotional situations, family dynamics, and in difficult environments.
* To work in partnership with parents/guardians and gain informed consent.
* To create individual care plans for specific children, negotiating, agreeing, implementing, and evaluating with parents and other professionals.
* To be responsible for assessing, delivering, developing, and maintaining high standards in the quality of care in the home and clinic setting, delivering face-to-face support and/or telephone advice.
* To work closely with the GPs and respiratory consultants to develop effective pathways of care and clinical management plans for patients.
* To maintain skills and training for procedures.
* Maintain an update log of all training undertaken.
* Identify and respond to issues of clinical risk.
* Provide information and training to parents based on assessment of need.
* Participate and support regular educational audits demonstrating examples on an annual basis.
* To assist in the education and practical development training of all learners, utilizing every opportunity for clinical teaching.
* To develop protocols and guidelines to ensure patient-centered, age-appropriate safe management for children and young people with asthma, including the transition to adult services.
* To recognize own professional limitations.
* Demonstrate the ability to manage time and resources effectively.
* Demonstrate an ability to prioritize workload and show flexibility in response to urgent and/or changing needs.
* Through reflection and clinical support, develop strategies to improve, and where necessary, change practice.
* Identify issues relevant to practice governance and participate in sharing knowledge in this forum.
* Identify areas of practice development and demonstrate the implementation of one practice development initiative per year.
In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £1,066 and a maximum payment of £1,845 per annum (pro rata where applicable).

Experience and Knowledge
* Knowledge and experience in Children's Community Nursing.
* Knowledge and experience of dealing with children with respiratory conditions.
* Knowledge and experience of teaching and assessing children, parents, and staff.
* Clinical assessment skills and experience.
* Sound knowledge of safeguarding issues and legal framework.
* Good understanding of asthma and the impact of this condition on the child and their families.
* In-depth knowledge and skills in the management of acute paediatric conditions and asthma.
Skills and Abilities
* Able to carry out nursing assessments independently.
* Observations and measurements using various medical devices: e.g., oxygen monitoring, urinalysis, vital signs including BP.
* Sound knowledge of medications, their dosing, and side effects.
* Care for the child or young person with complex health needs or learning disabilities.
* Knowledge and experience of how chronic illness affects the child, young person, and family.
