The Discharge Lounge at Pilgrim Hospital are looking for a Nursing Associate to join our multi disciplinary team which specialises in safe and effective patient discharges. Discharge Lounge (DL) provides an essential nurse led service in accepting patients on the day of discharge to release capacity for accommodation of acute admissions. DL supports timely and safe discharge of patients from inpatient wards, assessment units and the Emergency Department (ED). Thus improving patient flow through the emergency and planned care pathways by releasing inpatient bed and ED capacity. DL provides a safe and pleasant environment for up to 18 patients to be cared for whilst waiting for the final elements of their discharge/transfer to be completed. We operate single sex accommodation and are currently open 24 hrs a day, 7 days a week. Main duties of the job The nursing team is expected to deliver direct patient care, whilst co-ordinating the discharge of medically optimized patients. The role requires excellent communication and organisational skills and an ability to prioritise workload.This role is patient centric, an active interest in patient and family engagement is essential. The ideal candidate will have the ability to compassionately communicate with patients and family, ensuring a smooth transition from hospital to home or other specified destination; answering their queries and allaying or addressing any concerns.We are able to accept applicants for full and part time posts.
