The Memory Assessment Service in Cornwall Partnership NHS Foundation provides assessment and treatment to people living with dementia or suspected dementia.
We are looking for a passionate, highly motivated individual who is an effective communicator, with considerable experience in the role they are applying for, or experience in assessment of people with dementia. This post will work alongside the local complex care multi-disciplinary community teams, which form an essential part of the pathway from early diagnosis. The post is supported by the Dementia and Older People's Mental Health team, Primary Care Practitioners, Occupational Therapist, and the Dementia liaison nurse providing care for people across the pathway.
Main Duties of the Job
We are looking for a highly motivated individual, who is passionate about working in this field and who is looking to join an innovative service delivering high quality, safe and effective care, that people who use it say is good.
You will be a qualified professional and have experience of working with people with dementia and be able to manage a caseload placing the person at the centre of care in a rewarding but busy role. The ability to work in partnership with carers/family members and services in the community is essential.
We will provide you with management and clinical supervision and the opportunity to develop your skills in this area of work through training and development.
To provide specialist nursing skills, knowledge, and expertise in the assessment, care, and management of people with suspected dementia/dementia to support the Consultant, GP, and other primary care teams.
