Job summary The Prehabilitation service at CHFT aims to improve the outcomes of newly diagnosed cancer patients. The role will facilitate thedevelopment and delivery of a model of support using or adapting existing services and delivering the Active Together model to support the psychological wellbeing, dietary and physical activity needs of newly diagnosed cancer patients. Working in collaboration with partner organisations and Yorkshire Cancer Research the postholder will implement the Active Together prehab and rehab model. The Active Together programme aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare. The postholder will facilitate the development and implementation of Active Together, a 5-year funded programme with staged implementation into all relevant cancer pathways. The postholder will be a dynamic, motivated individual who is a self-starter, able to adapt to new strategies, ideas, and ways of working and they will strategically lead a team of staff and be expected to inspire the team to carry out a new and potentially challenging work programme. Excellent project management and organisational skills are required to ensure the project delivers outcomes both in place and across the locality.
