The main responsibilities of the role include: Delivering a high standard of patient care, using advanced, autonomous clinical skills and a broad, in-depth, theoretical knowledge-base Completing both face to face and telephone consultations Conducting home visits Triage Working collaboratively with the multi-disciplinary general practice team Providing expert advice to patients, carers and colleagues Developing innovative concepts, models, methods and practices, to help deliver improved primary care to our practice population Advancing patient health and well-being through the use of health promotion, health education, screening and therapeutic communication skills As an independent prescriber: to prescribe safe, effective and appropriate medication, as defined by current legislative framework Leading and undertaking research and audits Providing leadership, education and training to different healthcare professionals, staff and students As a vital part of our multidisciplinary team, you will have the opportunity to support and care for a diverse patient group. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: MSc/Degree in Advanced Nursing or RCN accreditation of Masters level study. Independent prescribing qualification. NMC registered. Advanced clinical assessment and examination skills. Management of patients with long-term health conditions. Management of patients with complex physical/psychological/social needs. Management of palliative and end of life patients. Experience as a nurse practitioner in general practice. Nurse-led management of minor illness, minor ailments and injuries. Reliable. Forward-thinking. Adaptable. Passionate about patient care. Highly motivated. Experience of SystmOne and ARDENS is desirable but not essential.
