Job summary The role of the Community Specialist Health Nurse is to provide holistic and effective nursing care to those people with learning disability requiring the support of Specialist Health Services. The post holder will liaise with the PCN leads within South Essex in order to progress the transformation of local services including the our key priority areas - including Health Coordination; frailty; enhanced physical health checks and; supporting the wider MDT to ensure that adults with learning disability have equitable access to mainstream health services and experience improved health outcomes. The Partnership was formed in 2018 and has been developing new pathways of working that seek to improve health outcomes for people with LD making sure that they are able to access the right care in the right place at the right time, in accordance with Building the Right Support.
