The individual’s will be based at Eva Burrows 1st Stop Project. Housing First is a new approach to supporting the most excluded and hard to reach rough sleepers. It focuses on finding permanent housing as quickly as possible and then addressing the issues that have contributed to an individual’s homelessness. Rather than follow normal routes of emergency accommodation to supported and temporary accommodation, Housing First offers people their own home and then gives them the intensive support needed to maintain their tenancies and move on with their lives away from the streets. Also, this team supports our Intensive Support Service Users within their own home, working with a range of needs that would support them to continue to maintain their own tenancy and integrate back into their community. Improve Access to Health and Social Care Services You will be responsible for the day-to-day delivery of the service, managing a caseload of clients with the overall goal of supporting them to access Health and Social Care Services, maintaining appointments and reducing the use of Out of Hours Services Provide tailored support to those using the service to meet individual need and aspiration, with people being able to choose, as far as possible, what, how and when support is delivered. To succeed you will need to have the ability to work independently and be comfortable as a lone worker, when required, with a good knowledge of the relevant protocols and safety precautions. You’ll be able to maintain enthusiasm for working with people with complex needs and provide the consistently high levels of support they require to meet their aims and objectives in a way that is flexible for everyone we support. The successful candidate(s) will be able to demonstrate and/ or hold: Level 3 qualification or equivalent in a related field or be willing to work towards Experience of working with vulnerable clients in programme area associated with the specialism Knowledge of the benefit system and an understanding of supported housing funding streams and reporting requirements Full Driving Licence In order to complete your application please download and read the job profile and any other attachments.
