Join one of our dynamic, committed and supportive integrated, multi-disciplinary Community Mental Health Teams in the thriving area of Trafford.
An opportunity has arisen to work in one of our busy, culturally diverse community mental health teams. The CMHTs are multidisciplinary teams, comprising of nurses, occupational therapists, psychiatrists, psychologists, social workers, employment specialists, and peer support workers. The teams are supported by good administrative systems and offer excellent working environments. The teams have good links with local statutory and non-statutory services. All the CMHTs work closely with our colleagues in Urgent care and the Inpatient Wards across GMMH.
Please note that this role is open to Band 5 RMNs with relevant physical health experience, as well as established Band 6 applicants.
Main duties of the job
This is an exciting position which will focus on the physical wellbeing of our service users with mental health needs. The post holder will be responsible for leading the physical health care agenda within the Trafford North area. You would fall under the supervision of the CMHT Manager and be supported by other senior practitioners on the team.
The post holder will ensure the delivery of all physical health care CQUIN requirements and trust key performance indicators. They will act as a specialist resource and advisor to care coordinators within the community teams. The post holder will work across community settings, including clinic bases and home visits.
The post holder will be required to work, for the majority of time, as an autonomous clinical practitioner for patient groups across a range of clinical settings. The post holder will be expected to forge strong links with the Care Coordinators, Physical Health Support staff, and Primary Care services, particularly GP practices.

Person Specification
Qualification
Essential
* Registered Mental Health Nurse.
* Substantial experience of working as an autonomous clinical practitioner.
Desirable
* Training in: Venepuncture.
* Experience in more than one specialism.
Knowledge
Essential
* Working knowledge and understanding of: Acute illness management and the associated monitoring of physical healthcare problems/conditions associated with serious mental illness.
Desirable
* Working knowledge and understanding of: Mental Health Act, Mental Capacity Act, and medicine management policies and NMC medicine management standards.
