We are looking for Band 6 Learning Disability Nurses for Community Specialist Health in Billericay, Essex starting ASAP for 20 hours a week for 12 weeks, between Mon-Fri 9-5.
As a Registered Learning Disability Nurse, you need to be a car driver with access to a car. The nurse will work as part of a multidisciplinary team providing support to individuals with learning disabilities who have complex physical health needs, challenging behaviour, and complex mental health issues. The role involves supporting these individuals to remain in the community while accessing appropriate healthcare. This also includes the uptake of annual health checks, support of end-of-life planning, review of DNACPR, and assistance with frailty. The person will need good networking and communication skills and be prepared to work autonomously. This is a community-based role.
