Your mission? To reach out to individuals with Dementia across Swansea and Neath, offering useful advice, information, practical support, and personal guidance through a holistic approach. By developing and implementing caregiver plans focused on the individual, you will play a central role in empowering them to navigate their journey confidently and resiliently. What does a day in the life of a Community Connector look like? You will: Develop, collaborate, and maintain positive and effective working relationships with partnership agencies throughout the community Engage with service users to identify specific needs and appropriate interventions Provide high-quality information, referral, and promotion to a wide range of services Support individuals to access local community services that suit their needs Work with individuals newly diagnosed with Dementia and their families What do you need to be a successful Community Connector? The ability to work as part of a team and on your initiative Knowledge of issues related to Dementia care Ability to plan your own workload Understanding of how to improve service quality for the benefit of users Willingness to work flexible hours if necessary Full clean driving license, with at least 2 years of driving experience and access to a vehicle The deadline for applications is December 31, 2024.
