RMN/RNLD/RGN Social Work - Health and Justice Custody Team (Worthing Custody Suite - Sussex)

You will work with marginalized patients as they encounter Police Custody as part of their criminal justice journey, collaborating with Police, Crown Prosecution Services, Probation, Mental Health services, and third sector agencies to achieve the best health outcomes for our service users.
MAIN DUTIES:
An exciting opportunity has arisen for a Band 6 Practitioner with a criminal justice interest. The Sussex Health & Justice Service operates in Police Custody across the locality, with this post based in Worthing.
The role includes:
1. Providing high-quality assessments and supporting individuals interacting with the criminal justice service.
2. Delivering services to people of all ages with vulnerabilities, including mental health, physical health, learning disabilities, substance misuse, unemployment, housing, and educational needs.
The successful applicants must be highly motivated, seeking a new challenge, and possess the skills to make a difference.
POLICE CUSTODY WORKING:
Key tasks include:
1. Arranging appropriate support for service users and liaising with professionals within the criminal justice system.
2. Completing assessments within custody.
3. Checking health records to aid triage and identify vulnerabilities.
4. Making onward referrals to services required to meet individual needs.
5. Ensuring all relevant agencies are informed of needs and risks for individuals.
6. Acting as a point of contact for professionals based in Police Custody.
7. Working as part of a team while having the freedom and ability to work independently.

Responsibilities include:
1. Taking responsibility for specialist assessment in Liaison and Diversion in conjunction with service users and carers, promoting self-determination, choice, and autonomy.
2. Providing written reports for Crown or Magistrates Courts or attending to give verbal evidence as required.
3. Participating in setting quality standards, including auditing, monitoring, and reviewing practices in line with current clinical guidance and policy.
4. Assessing patients' individual care needs and referring them to appropriate agencies.
5. Completing and contributing to robust risk assessments and risk management plans.
6. Maintaining an effective reporting system by observing and reporting on patient conditions.
7. Utilizing agreed outcome measures to review the effectiveness of processes as part of the Health and Justice Service.
8. Participating in the development of the service to ensure clinical governance processes are actively implemented at the team level.
9. Assisting in ensuring that the aims and objectives of the service (as set by the Team Leader) are fulfilled.
10. Assisting in implementing evidence-based practice and participating in research and other projects as required.
