Clinical Responsibilities: The post-holder is expected to take due note of and act in accordance with the standards of clinical governance that are agreed within the practice. Any concerns should be brought to the attention of the partners as soon as practicable. Act as an Advanced Clinical Practitioner demonstrating advanced knowledge and skills. To be a pivotal member of the Team. Organise the clinical management of individual patients, using highly developed expert clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate changes in management where required. This will involve the monitoring and evaluating the response to treatment through analysis and interpretation of clinical tests and investigations and the transfer of care/discharge of patients. Conduct advanced health assessments and clinical examination of patients with highly complex presentations and make clinical decisions based on these assessments. Work as an autonomous practitioner, management and prioritising own workload on a daily basis. Work to advise and recommend treatment for patients using clinical judgment, and decision making skills in accordance with agreed local, regional and national policies and guidelines. In partnership with other clinical teams, collaborate on improving the quality of healthcare responding to local and national policies and initiative as appropriate. Recognise and work within own competency and professional code of conduct as regulated by the NMC/HCPC. Produce accurate, contemporaneous and complete records of patient consultation. Assess effectiveness of care delivery through self and peer review, benchmarking and formal audit and evaluation in the quality governance systems and processes of the practice. Understand and apply legal issues that support the identification of vulnerable children and adults. To understand the statutory child/vulnerable health policy and procedures regarding domestic violence, vulnerable adults, substance abuse and addictive behaviour. To provide guidance and support to ensure appropriate referral if required. Ensure health and safety is always maintained. Work autonomously and to be effective and responsible for supporting others. Prioritise own workload and ensure effective time management strategies are embedded within the culture of the team. Work effectively with others. Contribute to the delivery of specifically identified services or projects. Awareness of and compliance with all relevant policies/guidelines including, but not limited to, prescribing, confidentiality, data protection, health and safety, QOF, CQC. Attend training and events organised by the practice or other agencies, where appropriate e.g. CPR training. This includes clinical and non-clinical training and events. Provide information when reasonably requested by the practice manager to ensure that all personnel and other records are accurate e.g. hepatitis status. To meet all deadlines for audits and written returns so that the practice meets quality standards and receives the designated funding (QOF, LES, DES etc.). The role will involve but not be limited to you performing the above.
