Job summary
Don Valley Healthcare are looking for an Advanced Clinical Practitioner to join our team. We welcome applicants who can actively contribute to both high quality patient care and cohesive team working. Who can also act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
Main duties of the job
This role will be suitable for an Advanced Clinical Practitioner who will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits across our 2 sites.
Within our team we currently have 1 DNP, 4 Practice Nurses, 3 HCA's and a phlebotomist.
Working hours are negotiable to preference, with flexibility to work Monday-Friday 8-6.30pm.
About us
We have a population size of approx 14,000 over 2 sites in Bentley and Sprotbrough. We have 8 GP partners and 5 salaried GP's and a full complement of clerical administrators and receptionists.
We are a training practice for Registrars, Student Nurses and Medical Students and are also a research ready practice and actively take part in research projects.
Job description
Job responsibilities
1. To practice autonomously within the designated speciality area to provide patient-centred clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the directorate.
2. Work autonomously as an advanced practitioner within the specialty, managing a caseload of patients delivering individualised direct patient care.
3. Direct responsibility for assessment, examination, investigation and diagnosis of patients within their area of work.
4. Appropriately treat patients, resulting in the sage management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations.
5. Undertake a variety of clinical skills and provide treatment/advice as per speciality and scope of practice using agreed protocols of clinical practice and professional guidelines. This service may be provided within community/home care settings.
6. Within scope of practice and clinical competence, request and/or undertake diagnostic procedures and clinical investigation related to plans of care.
7. Utilise scope of practice and provide on medicine management issues associated with the patient specialty group.
8. Provide a seamless, high quality service from referral through to assessment, diagnosis, treatment and review, referring to other specialists as required.
