Deben Health Group PCN is looking to welcome a qualified and experienced pharmacist technician into our busy practices, to play a full and active role within our multi-disciplinary team. The role will be based from Riduna Park, Melton and Woodbridge but you will be required to travel across all surgeries within the PCN. This position is offered on a full or part-time basis depending on the individuals circumstances. Pharmacy technicians play an important role, complementing and working alongside the PCN clinical pharmacists, community pharmacists and other members of the PCN multi-disciplinary team. Main responsibilities of the role include; Undertaking patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients. Carrying out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), actioning, clinical letters and discharge summaries, supporting medication reviews, medicines reconciliation and drug monitoring. As determined by the PCN, supporting medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacies. Supporting the clinical pharmacist in structured medication reviews (SMR) e.g. organise necessary monitoring tests prior to SMR. Providing expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities. Managing shared care protocols and liaise with clinical pharmacists for more complex patients. Supporting initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally. Please see job description attached for further details. About the Candidate The successful candidate will have the following experience, qualifications and skills; Registered with General Pharmaceutical Council (GPhC). BTEC/NVQ level 3 or equivalent in pharmaceutical sciences. Experience of working as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy. Confident in use of electronic records and systems; databases and spreadsheets. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing. Excellent written and verbal communication skills (Email, telephone & online meetings).
