Chronic Disease Management, carrying out chronic disease clinics following practice protocols, including COPD, Asthma, CHD, Stroke, Hypertension and Diabetic. Health promotion, recognising and supporting patients readiness to change unhealthy behaviour. Providing healthy eating, exercise, alcohol and smoking cessation advice and clinics. Vaccinations and Immunisations, providing an adult and childhood immunisation service. Maintain a call and recall of patients. Develop and maintain practice protocols. Travel Health Advice, offering a holistic approach to travel health including comprehensive advice for patients prior to travel. Ear care, advising patients about safe ear care and if necessary undertaking ear syringing. Family Planning and Womens Health, providing a comprehensive family planning and sexual health service to patients. Undertaking a programme of cervical cytology. Complex wound management, providing the diagnosis, management plan and treatment of leg ulcers using Doppler assessment. Infection control, supporting patients and colleagues in adopting sound infection control measures. Applying infection control measures in the practice according to local and national guidelines.
