Your role purpose is to deliver quality, efficient patient centred care by engaging in comprehensive remote patient management.
Engaging in comprehensive remote patient management
* Make outbound calls to patients who are unwell, assessing the severity of their condition, providing initial advice, supporting emotional needs, and guiding on immediate steps to manage symptoms or concerns.
* Signpost unwell patients to appropriate services, resources, or professionals, facilitating connects where needed.
Adhering to regulatory compliance
* Adhere to all established standards, policies, and protocols to ensure safe and effective patient care.
* Maintain strict confidentiality of patient information.
* Uphold ethical standards, professional codes of conduct, and all relevant legal requirements governing healthcare practice.
Engaging in professional development and contributing to service improvement
* Develop and maintain clinical skills and knowledge as necessary.
* Engage in ongoing professional development activities to perpetually enhance clinical knowledge and skills.
* Actively participate in quality improvement initiatives, including engaging in surveys and audits, to enhance the quality of patient care and services.

About You
The ideal Candidate will be an Active Registered Nurse (RN) with current NMC registration.
* Clinical knowledge in oncology.
* Communication skills conveying information clearly and empathetically.
* Understanding of healthcare regulations, guidelines, and best practices.
* Knowledge of ethical and legal practices in nursing with a particular focus on patient privacy and data protection.
* Strong verbal and written communication skills, especially for interacting with patients, families, and other healthcare providers.
* Building rapport and trust with patients and caregivers.
* Knowledge of relevant healthcare laws, ethical guidelines, and experience ensuring compliance, particularly in documentation and maintaining confidentiality.
