Main duties of the job To undertake an assessment of housing and social needs of individuals with mental health difficulties who are in-patients on mental health wards within the county area of Nottinghamshire Healthcare NHS Foundation Trust. To act as a lead in relation to addressing identified housing and social difficulties. To pro-actively engage within bed management meetings across the Trust, working in collaboration with statutory and non-statutory organisations to reduce hospital in-patient stays for patients and thus contribute to the reduction of delayed transfer of care (DTOC). To act as a resource for social and housing difficulties and related issues across the acute community services.
