Job summary We are looking for a positive, dynamic and motivated band 5 nurse to work as part of the care coordination team in all Claire House settings; Wirral Hospice, Liverpool Hospice and the community setting. The individual will be capable of working across teams and services internally and externally. This role is full-time and can be worked flexibly across 4 days (a minimum of 30 hours is required), predominantly office hours, with an expectation to work flexibility over a 7-day period if needed, with an option to work at home at times; dependent on service need. We are looking for post-registration experience and knowledge within nursing care (pediatric/young adult environment). This role involves supporting families holistically within their journey at Claire House childrens hospice. This role will be a main point of contact for families and professionals to build positive relationships with increased holistic support in times of need and ability to signpost when appropriate. To practice and promote a high level of specialist palliative care, encompassing a supportive and holistic approach to the child/young person and their families with a life-limiting condition.
