This is a unique opportunity to join a team committed to empowering adults with learning disabilities, epilepsy, acquired brain injuries, and complex mental health needs to live fulfilling and independent lives.
Your Impact:
In this vital role, you'll provide person-centered support, fostering independence and enhancing the well-being of individuals within a residential supported living setting. Your contributions will directly impact their quality of life, enabling them to thrive within their community.
Key Responsibilities:
* Holistic Support: Deliver individualised care plans that address the unique needs of each service user, encompassing physical, emotional, and social well-being.
* Daily Living Assistance: Provide sensitive support with personal care, medication administration, meal preparation, and household tasks, promoting dignity and autonomy.
* Community Integration: Facilitate meaningful engagement in community activities, appointments, and social events, fostering inclusion and reducing isolation.
* Mental Health Support: Provide empathetic support to individuals experiencing mental health challenges, working alongside professionals to promote emotional well-being.
* Accurate Documentation: Maintain detailed and accurate records, including daily logs, incident reports, and support plan updates.
* Collaborative Teamwork: Work effectively as part of a multidisciplinary team, collaborating with families, healthcare professionals, and other stakeholders.
* Flexible Scheduling: Adapt to a flexible work schedule, including weekends, evenings, and sleep-in duties.
* Proven experience supporting adults with learning disabilities, epilepsy, and acquired brain injuries.
* Demonstrated understanding and experience in supporting individuals with mental health needs.
* Exceptional communication and interpersonal skills, fostering positive relationships with service users and colleagues.
* Previous experience in care assistance, social care, or working with vulnerable adults.
* Ability to work independently and as part of a collaborative team.
* Commitment to providing high-quality, person-centered care.
* Willingness to participate in ongoing training and professional development.
* A NEW enhanced DBS check will be required.
Desirable Skills & Qualifications:
* Relevant qualifications in health and social care.
* MHFA (Mental Health First Aid) Certification
* Full UK driving license.
